PA Board of Barber Examiners Centre County Thomas Tubbs of Bellefonte, Centre County, had his application to sit for the barber licensure examination granted and, upon successful completion of the examination, he shall be granted a restricted license for a period of two years based on his criminal conviction. (5/27/2021) Cumberland County James Robinson, III, of Mechanicsburg, Cumberland County, had his application to sit for the barber licensure examination granted, and upon successful completion of the examination, his barber license shall be granted and then immediately placed on probation for two years based on his criminal conviction. (06/15/21) Lancaster County Quincy Kareem Parks of Lancaster, Lancaster County, had his application to sit for the barber licensure examination granted and, upon successful completion of the examination, his barber license shall be granted and then immediately placed on probation for one year based on his criminal conviction. (5/18/2021) Northampton County Joseph Luis Ramos of Bethlehem, Northampton County, had his application to sit for the barber manager licensure examination granted and, upon successful completion of the examination, his barber manager license shall be granted and then immediately placed on probation for two years based on his criminal conviction. (06/11/21) Philadelphia County Josephita Brown, of Philadelphia, Philadelphia County, had her application to sit for the cosmetologist licensure examination granted and, upon successful completion of the examination, her cosmetologist license shall be granted and then immediately placed on probation for two years based on her criminal conviction. PA Board of Landscape Architects No disciplinary actions taken. PA Board of Massage Therapy Lancaster County Gregory A. Gilson, L.M.T., license no. MSG003370, of Lititz, Lancaster County, had his license suspended based on his unprofessional conduct, his violation of the standards of professional practice, and a sexual violation in the practice of massage therapy. (6/7/21) Philadelphia County David Steven Alder, license no. MSG008639 of Philadelphia, Philadelphia County, permanently voluntarily surrendered his license to practice as a massage therapist, and was levied a civil penalty of $500; because he violated the standards of professional practice or conduct as established by board regulation by committing sexual violation. (6/1/21) PA Board of Medicine Allegheny County Joseph Nicholas Dicroce, license no. MD032169E of Monroeville, Allegheny County, was indefinitely suspended, immediately stayed in favor probation until the resolution of his pending criminal court case, because he failed to conform to a quality or ethical standard of the profession. (5/25/21) Berks County Fred Michael Michalac, license no. YM014718 of Reading, Berks County, was suspended for no less than three years, stayed in favor of probation, because he is unable to practice the profession with reasonable skill and safety to patients by reason of illness, addiction to drugs or alcohol, or mental incompetence. (5/25/21) Cambria County Peter James Ridella, license no. MD018887E of Johnstown, Cambria County, permanently voluntarily surrendered his license, because he was convicted of two felony offenses in a federal court. (5/25/21) Delaware County Dominique Raquel Etienne, license no. MA056503 of Drexel Hill, Delaware County, was indefinitely suspended until such time as her pending criminal case has been resolved, because she failed to conform to the ethical and quality standard of the profession (5/25/21) Luzerne County Page 5 of 15
Kerry Ira Weiss, license no. MD039280L of Bear Creek Township, Luzerne County, must complete 15 hours of continuing medical education on the topics of record keeping and/or informed consent, because he failed to conform with the acceptable quality standard of the profession in that he completed a circumcision on a baby without the informed consent to do the circumcision by the baby's parents. (5/25/21) David Jacob Shiling, license no. MD460041 of Kingston, Luzerne County, received a reprimand on hie disciplinary record with the Board, because he had a license or other authorization to practice the profession disciplined by the proper licensing authority of another state (5/25/21) Out of State Tolulope Kofi Akinyemi, M.D. of Washington, D.C., had his license to practice as a physician and surgeon indefinitely suspended based upon a violation of a Board Order. (6/22/21) Navigation Commission for the Delaware River and its Navigable Tributaries No disciplinary actions taken. Secretary of the Commonwealth – Notaries Public Lycoming County Deeann M. George, notary commission no. 1057485, of Williamsport, Lycoming County, was suspended for 24 months, immediately stayed in favor of probation, must complete six-hours of notary practice and procedure course(s), was levied an administrative penalty in the amount of $2,000; and probation assessment fees in the amount of $1,200; because she failed to require the personal appearance of two individuals on a specific occasion, failed to require personal appearance generally, failed to maintain a complete notary journal with all required information, and maintained an electronic journal in a non-tamper-evident format. (6/3/21) Philadelphia County The notary public commission of Kristen Borman (ID 1301925), of Philadelphia, Philadelphia County, is revoked, based on her failure to require the personal appearance of a document signer on several occasions; failure to securely maintain her notary stamping device; failure to notify the Department that her notary stamp was lost or stolen; allowing another individual to use her notary stamp; failure to record in her notary register the character of the notarial act performed and receiving fees for her notary services in excess of the fees fixed by the Secretary. The notary public commission of Cesar Molina (ID 11281958), of Philadelphia, Philadelphia County, is revoked, based on his 1) failure to maintain security over his stamping device by keeping his seal in a box in an easily accessible spot in his workplace and from which the stamp could be accessed by any other person and 2) failure to properly maintain a journal of notarial acts, by not recording the time of the notarial act; a description of the type of notarial act performed; the issue date of the form of identification used to verify customer identities; and the fee charged. PA Board of Nursing Allegheny County Jennifer N. Legg, license no. RN618158, of Pittsburgh, Allegheny County, was reinstated to probation based on compliance with a previous board order. (06/3/21) Tiffany Anne Hafner, license no. RN703957 of Pittsburgh, Allegheny County, was indefinitely suspended for no less than three years, retroactive to April 23, 2020, immediately stayed in favor of probation, because she is unable to practice professional nursing with reasonable skill and safety by reason of mental or physical illness, condition, or dependence upon alcohol, hallucinogenic or narcotic drugs, or other drugs which tend to impair judgment or coordination. (6/10/21) Bucks County Geraldine A. Fitzpatrick, RN, license No. RN319548L, of New Hope, Bucks County, had her license automatically suspended, effective May 10, 2021, for her misdemeanor conviction under the Controlled Substance, Drug, Device and Cosmetic Act. (06/22/21) Amber Dawn Parker, RN, license no. RN620122, of Hulmeville, Bucks County, had her license indefinitely suspended and was assessed $500 in costs of investigation because she is unable to practice with reasonable skill and safety to patients and for violating a Board Order. (06/21//21) Butler County Celli Ann Wisneski, license no. RN527572L of Saxonburg, Butler County, was indefinitely suspended for no less than three years, immediately stayed in favor of probation, because she is unable to practice professional nursing with reasonable skill and safety to patients by reason of mental or physical illness, condition, or physiological or psychological dependence upon alcohol, hallucinogenic or narcotic drugs, or other drugs which tend to impair judgement or coordination. (6/10/21) Clearfield County Kimberly Dawn Olson, license no. PN300459 of Dubois, Clearfield County, received a reprimand on her disciplinary record with the Board, because she poses a substantial risk to the health and safety of the individual’s patients or clients or the public, or a substantial risk of further criminal convictions. (6/10/21) Cumberland County Robert Michael Garofalo, license no. RN623660 of Lemoyne, Cumberland County, was indefinitely suspended for no less than three years, immediately stayed in favor of probation, and levied costs of investigation of $385; because he is unable to practice the profession with reasonable skill and safety to patients by reason of mental or physical illness, condition, or physiological or psychological dependence upon alcohol, hallucinogenic or narcotic drugs, or Page 7 of 15
other drugs which tend to impair judgement or coordination, and was convicted, entered a plea of guilty, entered a plea of nolo contendere, or has been found guilty by a judge or jury of a felony or a crime of moral turpitude, or has received probation without verdict, disposition in lieu of trial or an accelerated rehabilitative disposition in the disposition of felony charges, in the courts of this Commonwealth, the United States or any other state, territory, possession or country. (6/10/21) Dauphin County Lisa A Finefrock, license no. PN250258L of Harrisburg, Dauphin County, was indefinitely suspended for no less than three years, immediately stayed in favor of probation, because she is unable to practice the profession with reasonable skill and safety to patients by reason of mental or physical illness, condition, or physiological or psychological dependence upon alcohol, hallucinogenic or narcotic drugs, or other drugs which tend to impair judgement or coordination. (6/10/21) Delaware County Brittany Ann Pierdomenico, LPN, No. PN284959, of Marcus Hook, Delaware County, had her license suspended for no less than one year, based upon having pled guilty to felonies and a misdemeanor, committing fraud or deceit in the practice of nursing, misappropriating property or money from a patient and being guilty of unprofessional conduct. (4/29/21) Jeannie M. West, RN, license no. RN571954, of Hummelstown, Dauphin County, was suspended for no less than three years, retroactive to March 15, 2021, based on violating the terms of a previous Board order. (06/7/21) Daniel James Datillo, license no. RN689858 of Glen Mills, Delaware County, was indefinitely suspended, immediately stayed in favor of indefinite probation, because he is unable to practice professional nursing with reasonable skill and safety by reason of mental or physical illness, condition, or dependence upon alcohol, hallucinogenic or narcotic drugs, or other drugs which tend to impair judgement or coordination. (6/10/21) Erie County Pamela Ann Williams, LPN, license no. PN297718, of Erie, Erie County, was suspended for no less than three years, retroactive to July 25, 2019, based on violating the terms of a previous Board order. (05/27/21) Nancy R. Farrell, license No. PN271984, of Erie, Erie County, had her license revoked for violating the terms of a prior Board Order. (05/05/21) Franklin County On May 7, 2021, the State Board of Nursing publicly reprimanded and assessed a civil penalty of $250 upon the license of Janet Michelle Yeager, license no. RN578624, of Greencastle, Franklin County. The Board took this action based on disciplinary action being taken against her Page 8 of 15
license by the proper licensing authority of another state and her failure to report same to the Board. Greene County Shana Marie Ferguson, license No. RN679827, of Carmichaels, Green County, had her license suspended retroactive to March 15, 2021 for violating the terms of a prior Board Order. (06/03/21) Lancaster County Lillian Akello Ogwang, license no. PN283442, of Lancaster, Lancaster County, was suspended for no less than three years retroactive to November 7, 2020, until respondent is fit and competent to resume the practice of practical nursing with reasonable skill and safety to patients. At least six months of this suspension shall be active. (05/5/21) Lindsey Marie Cox, license no. PN308658, of Ephrata, Lancaster County, had her license suspended because she violated a previous board order. (05/13/21) Tina Williams, license no. RN548085 of Landisville, Lancaster County will be indefinitely suspended for no less than three years, immediately stayed in favor of probation, because she is unable to practice professional nursing with reasonable skill and safety by reason of mental and physical illness or condition or dependence upon alcohol, hallucinogenic drugs or other drugs which tend to impair judgment or coordination. (6/10/21) Lehigh County Kathleen B Middleton, license no. RN534824 of Allentown, Lehigh County, voluntarily surrendered her license, because she is unable to practice professional nursing with reasonable skill and safety to patients by reason of mental or physical illness, condition, or physiological or psychological dependence upon alcohol, hallucinogenic or narcotic drugs, or other drugs which tend to impair judgement or coordination, so long as such dependence shall continue. (6/10/21) Luzerne County Donald W. Muendlein, license no. RN676059, of Wapwallopen, Luzerne County, had his license indefinitely suspended and was assessed costs of investigation because he is unable to practice with reasonable skill and safety to patients and for violating a Board Order. (06/16/21) Montgomery County Stanley C Martin, RN, No RN535211, of Horsham, Montgomery County, had his license indefinitely suspended and is permitted to seek reinstatement to probation after one year of active suspension through his criminal probation and upon completion of three hours of continuing education in communication for having pled guilty to two misdemeanor crimes of moral turpitude. Pamela Ann Tashjian, license no.PN066145L of Telford, Montgomery County, voluntarily surrendered her license to practice a Practical Nurse in the Commonwealth of Pennsylvania because she is addicted to alcohol and hallucinogenic or narcotic drugs or drugs which tend to impair judgement or coordination or has made her become mentally incompetent (6/10/21) Northumberland County Lucas Thomas Vaughan, license no. PN294566, of Mount Carmel, Northumberland County, had his license indefinitely suspended retroactive to February 17, 2021, and was ordered to pay costs of investigation, because he is unable to practice nursing with reasonable skill and safety to patients and for violating a Board order. (04/29/21) Kathy J Gorton, license no. RN510222L of Watsontown, Northumberland County, received a public reprimand and was ordered to complete at least six hours of remedial education in the areas of patient confidentiality, HIPPA and Ethics because she willfully or repeatedly violated the regulations of the Board when she failed to adhere to the standards of nursing conduct by failing to safeguard the patient's dignity, the right to privacy and confidentiality of patient information (6/10/21) Philadelphia County Tian Nikeisha Henderson, license no. PN280343, of Philadelphia, Philadelphia County, had her license reinstated to probationary status, for no less than two years, based on her compliance with a previous board order. (05/13/21) Jemirah Thompson, License No. RS358294, of Philadelphia, Philadelphia County, was granted a probationary license through completion of criminal probation and payment of all costs, fines, and/or restitution. (05/12/2021) Marijane Kling, license no. RN219615L of Philadelphia, Philadelphia County, voluntarily surrendered her license to practice as a registered nurse, because she is unable to practice professional nursing with reasonable skill and safety to patients by reason of mental or physical illness or condition or physiological or psychological dependence upon alcohol, hallucinogenic or narcotic drugs or other drugs which tends to impair judgment or coordination (6/10/21) Sylena Dawn Tackett, LPN, license no. PN303455, of Philadelphia, Philadelphia County, was suspended for no less than three years, retroactive to March 15, 2021, based on violating the terms of a previous Board order. (06/22/21) Schuylkill County Shannon Marie Tucker, LPN, license no. PN273028 of Frackville, Schuylkill County, had her practical nurse license indefinitely suspended, with the first six months to be served as active suspension and assessed a $1,000 civil penalty, based on her conviction of crimes which directly relate to the nursing profession and failure to report same to the Board. Jacqueline Costy, license no. PN280279, of Pottsville, Schuylkill County, had her license suspended, retroactive to April 14, 2021, because she violated a previous board order. (06/08/21) Tioga County Jessica Leigh Gesford, LPN, license No. PN278883, of Wellsboro, Tioga County, had her license indefinitely suspended retroactive to May 2, 2021, and was ordered to pay costs of investigation in the amount of $350 for being unable to practice nursing with reasonable skill and safety to patients, and for violating a Board order. (06/22/21) Westmoreland County Michael Anthony Maciupa, RN, No. RN686828, of Irwin, Westmoreland County, had his license revoked for having pled guilty to two misdemeanor crimes of moral turpitude. (6/15/21) Out of State Audelia Del Carmen Wallace, RN, PA license no. RN572128, of Hagerstown, Maryland, was indefinitely suspended based on being disciplined by the proper licensing authority of another state and failing to report the same to the Board. (05/04/21) Amy Michelle Murphy, RN, CRNP, PA License No. RN318522L and PA Certificate No. SP018180, of Brownwood, Texas, had her license and certificate reprimanded and a $250 civil penalty imposed for having been disciplined in another state and failing to report the same to the Board. (5/11/21) Beth Mackfee, RN, No. RN588548, of Pemberton, NJ, Burlington County, had her license indefinitely suspended for having been disciplined by the proper licensing authority of another state. (6/15/21) Jack M Williams, license no. RN318785L of E Liverpool, OH voluntarily surrendered his license to practice as a professional nurse in the Commonwealth of Pennsylvania, because he had a license suspended, revoked, or has received other disciplinary action by the proper licensing authority in another state, territory, possession, or country, and is guilty of immoral or unprofessional conduct by utilizing adult sex devices in a patient’s home and then leaving those devices in the patient’s home. Bucks County Richard Alan Kondan, license no. OS007466L of Doylestown, Bucks County, was indefinitely suspended effective July 1, 2021 and shall remain in effect until the resolution of his pending criminal case in Bucks County, because he failed to conform to the standards of acceptable and prevailing osteopathic medical practice. (6/9/21) Delaware County Patricia L Sutton, license no. OS005269L of Norwood, Delaware County, was restricted in his practice and licensure until the completion of at least a four-week preceptorship under a LifeGuard-approved preceptor, after which time the preceptor shall submit a final report indicating the completion of the preceptorship at which time her license shall be reinstated to probationary status for no less than one year, because he violated a regulation promulgated by the board or an order of the board previously entered by it in a disciplinary proceeding. (6/9/21) Jason L Delcollo, license no. OS015963 of Glen Mills, Delaware County, was suspended for 30 days, and levied costs of investigation of $1,347.75; because he committed a sexual boundary violation with a patient. (6/9/21) Montgomery County Lawrence I Miller, license no. OS009548L of Lansdale, Montgomery County, was restricted to practice solely in an active-retired status, because he was convicted of a felony in the courts of this Commonwealth, a Federal Court, or a court of any other state which is a criminal offense the Board has deemed as directly related to his occupation, trade or profession and as such he poses a substantial risk to the health and safety of his patients or the public or a substantial risk of further criminal convictions. (6/9/21) PA Board of Pharmacy Delaware County Gregory Dale Malloy, license no. RP039080L of Folcroft, Delaware County, was suspended until his license is released from suspension or probation in Delaware, immediately stayed in favor of probation, because he had his license to practice pharmacy suspended revoked or refused, or received other disciplinary action by the proper pharmacist licensing authority of another state, territory, or country. (6/8/21) Lancaster County Jeffrey W. Eshelman, license no. RP028441L of Lititz, Lancaster County, voluntarily surrendered his license and registration to practice as a pharmacist in the Commonwealth of Pennsylvania, because he pled guilty to felony offenses in connection with the practice of pharmacy (6/8/21) Philadelphia County Page 12 of 15
Neighborhood Pharmacy & Brad Silver, permit no. PP481924 and RP447957 of Philadelphia, Philadelphia County, was levied a civil penalty of $5,000; costs of investigation for $4,035.42; and must complete a three hour course entitled, “Pharmacy Law,” because Neighborhood Pharmacy, by and through its agents and employees, and Brad Silver, as his role as pharmacy manager, both failed to safeguard the storage and distribution of prescription drugs and devices under Commonwealth and Federal statutes, maintain proper records and advise, upon request, concerning contents, therapeutic values and use of drugs, devices and articles. (6/8/21) Out of State Bet Pharm LLC, registration no. NP000157 of Lexington, KY was suspended until September 1, 2025, immediately stayed in favor or probation, and levied a civil penalty of $4,000; because they had their permit to conduct a pharmacy suspended or revoked or the nonresident pharmacy was otherwise disciplined by the proper licensing authority of another state. (6/8/21) Purescience Rx, registration no. NP001306 of Poway, CA was levied a civil penalty of $2,000; because they had a permit to conduct a pharmacy suspended or revoked or the nonresident pharmacy was otherwise disciplined by the proper licensing authority of another state. (6/8/21) PA Board of Physical Therapy Lackawanna County Bradley Keith Macmaster, certificate no. TE011824 of Carbondale, Lackawanna County, permanently voluntarily surrendered his license to practice, because he was convicted of Corruption of Minors. (5/27/21) Lawrence County Jodi Pensy, PTA, certificate no. TEI000758, of New Castle, Lawrence County, had her certificate suspended for no less than three years based on her failure to abide by the terms of a previous Board Order. (5/28/21) PA Board of Podiatry No disciplinary actions taken. PA Board of Psychology No disciplinary actions taken. Real Estate Commission Philadelphia County McKinley Brown, real estate salesperson, license no. RS341990, of Philadelphia, Philadelphia County, was suspended pursuant to the Order of the Court of Common Pleas of Philadelphia County dated May 12, 2021, which the court issued under section 4355 of the Domestic Relations Code. Shaulis, license number MV219041, of Berlin, Somerset County, had his license suspended for a period of three years, stayed in favor of probation, based upon his criminal convictions. the topic of veterinary medical recordkeeping, because she departed from standards of acceptable and prevailing veterinary medical practice by failing to conform to the Board's recordkeeping regulations. (5/21/21) Denis Lee Daman, license no. BV004404L, of Butler, Butler County, had his license to practice veterinary medicine temporarily suspended, pending a hearing, on the grounds that his continued practice within the Commonwealth may be an immediate and clear danger to the public health and safety.
