P r e s i d e n t ’ s M e s s a g e 2020 The 27th Council has completed its first year of office, and will be continuing another year to complete its term of office. Work of the 27th Council (2019-2021) 2020 has been a challenging year since Covid-19 landed on our fair shores. The College has been very much at the forefront, helping our doctors manage the onslaught of the novel coronavirus. For many of us, we were reminded of SARS all those years ago; thankfully, this time the College and the primary care community were very much better prepared and coordinated. With strong support from the Ministry of Health, the supply of personal protective equipment enabled our doctors in clinics across the island to provide much needed essential services at the front line. Our team of dedicated volunteers also rose to the challenge and have been involved in various committees and task forces. As we continue to learn more about the characteristics of the virus, we have had to refine how we manage our patients on a day-to-day basis. There have been many developments and the College has been in the thick of them, representing the interests of the family physicians and GPs. All this could not have been achievable but for the leadership and volunteer spirit of our 27th Council, who have admirably stepped up to the plate and performed very well. Membership The membership of the College as of 31st March 2020 is 2534, increased from 2402 in 2019. We are heartened by the steady increase in membership over the years, giving us confidence to make better and stronger representation for the primary care community, and advocate our vision for the future of family medicine in Singapore. 15 new Fellows were admitted in 2019, taking the total number of Fellows to 195. There are now 668 qualified with MMed(FM), an additional 48. GDFM holders now stand at 1326, with 179 passing in 2019. The College will continue to train and support our family physicians in their training through our various programmes in family medicine. Assets College continues to be in good financial standing, giving us as an institution the security and confidence to continue the mission of our College. However, we must continue to remember that the College is a not-for-profit organisation and does not receive any government funding. Prudent financial management is key to preserving the resources the College needs to advance the cause and practice of family medicine. Courses conducted The GDFM, MMed(FM) College Programme and Fellowship of the College of Family Physicians Singapore (By Assessment)(FCFPS) are the 3 key courses we run for postgraduate family medicine, and they continue to do well. The curriculum and delivery of these programmes are being constantly reviewed and improved over the years. With the arrival of Covid-19 in January this year, the team worked furiously to implement distance learning by webinars, and this has been greeted with much enthusiasm. It has been an unmitigated success, with very large attendances for our events. Going forward we aim to improve and evolve the courses to better meet the needs of our trainees and members. Partnerships The 3 medical professional bodies (PBs), the College, the Singapore Medical Association and the Academy of Medicine Singapore, continue to work closely on various projects affecting healthcare in Singapore. In 2019, the 3 PBs were appointed members of the Review Workgroup for SMC disciplinary process and informed consent-taking standards. The workgroup met with all sectors of our healthcare fraternity, gathering feedback from over 30 townhall sessions and 1000 doctors. After 9 months of gestation, the long awaited report by the Review Workgroup was finalized and released in December 2019. The recommendations were wide ranging and its implementation is in the pipeline. The College continues to have an excellent excellent working relationship with the Ministry of Health. We work closely with the various divisions of the Ministry of Health (MOH), such as the Primary and Community Care Division, the Agency for Care Effectiveness and the Agency for Integrated Care, as well as the Singapore Medical Council, on various projects involving family medicine issues. This engagement has borne much fruit, as evidenced by programmes and policies that are now more primary care and GP-centric. 4 College of Family Physicians Singapore ANNUAL REPORT 2019-2020
Our 27th Council Members have given much of themselves and worked tirelessly for you and the fraternity. Our academic faculty have served selflessly in our training programmes, we are proud and grateful for their commitment and dedication to teaching and mentoring. They are truly an inspiration! Our very hard working and efficient Secretariat have also performed superlatively well over the past year, especially the extra work they've had to do to keep the College programmes going at this challenging time. We are indeed grateful for their dedication and are very proud of their efforts. The Albert and Mary Lim Award The Albert and Mary Lim award first presented in 1974 in memory of the late Dr Albert Lim Liat Juay and his wife Mary Lim was re-instituted in 1999. This award is the highest accolade bestowed by the College for contribution and services rendered to the College and the discipline of family medicine. Koh Eng Kheng Gold Medal The Koh Eng Kheng Gold Medal was established under the kind donation of the late Dr Koh Eng Kheng’s family. The Medal would be awarded to the Best Fellowship Candidate of each academic year. CONTINUING PROFESSIONAL DEVELOPMENT / MEDICAL EDUCATION PROGRAMMES AND COURSES The College promotes Continuing Professional Development (CPD) and Continuing Medical Education (CME) through the Family Medicine Modular Course, Family Practice Skills Course, MMed (Family Medicine) College Programme, Family Medicine Membership and Fellowship Programme. The Institute of Family Medicine (IFM), an organ within the College, plays a significant role in developing the contents and syllabi of these programmes and courses. The Covid-19 pandemic has resulted in much changes in the way the College functioned from February 2020 onwards. All the three College training programmes as well as the CPD activities had to be converted to an online based medium in face of the social distancing measures and the circuit breaker circumstances. This was successfully done and there were minimal disruptions to the administrative and operational processes. During the year the IFM reviewed, updated and edited the notes as well as their e-learning components for Modules 8, 1, 2 and 3 of the Family Medicine Modular Course to make them more relevant and user-friendly to the course participants. The Family Medicine Modular Course consists of 8 modules of knowledge acquisition and is a requirement for doctors who are preparing for the Graduate Diploma in Family Medicine (GDFM) Programme and the Master of Medicine (Family Medicine) College Programme. Each year, four modules are covered. For the year April 2019 - March 2020, there were 222 trainees registered in the GDFM course, 36 trainees in the MMed(FM) College Programme and 18 trainees in the Fellowship [FCFP(S)] programme. Family Practice Skills Course (FPSC) During this financial year, the College conducted 8 Family Practice Skills Courses. Assessments which can be written, oral, demonstration and/or clinical (for some courses) A typical FPSC template is a course with six units of distance learning with 30 MCQs test, conducted in two seminars, and also workshops conducted over two afternoon sessions typically, one on Saturday afternoon and one on Sunday afternoon. E-Learning The E-Learning programme has been incorporated into the GDFM training programme since 2003 consisting of therapeutic notes, case scenarios, assessment by MCQs and a case review.. Due to the Covid-19 situation, from February 20 onwards, the entire GDFM programme was conducted on-line including the workshops and the tutorials. The MMed(FM) and Fellowship programmes were also similarly switched to on-line teaching and training. Accreditation of core CME points The College is represented by Adj Asst Prof Tan Tze Lee in the Singapore Medical Council (SMC) CME Co-ordinating Committee. The College works closely with the SMC to ensure that the quality of the CME events is acceptable. It continues to advise SMC on the accreditation of core Family Medicine CME events through its panel of Family Medicine (FM) CME advisors appointed by the College. SIGNIFICANT COLLEGE EVENTS Annual General Meeting 2019 The College AGM was held on 27 July 2019 at Academia SGH. The minutes have been circulated. Commencement of Academic Year 2019 The Commencement Ceremony was held on 27 July 2019 at Academia SGH. There were 222 trainees in the GDFM course, 36 trainees in the MMed(FM) College Programme and 18 trainees in the FM Fellowship [FCFP(S)] Programme. Convocation 2019 The Family Medicine Convocation Ceremony was held on 23 November 2019 at Sofitel Singapore, Sentosa Resort and Spa and the Guest-of-Honour was Mr Edwin Tong, Senior Minister of State, Ministry of Law and Health. 19 doctors were conferred the MCFP(S). GRADUATE DIPLOMA OF FAMILY MEDICINE (GDFM) The Graduate Diploma in Family Medicine (GDFM) is a vocational training certification for primary care doctors. The aim of this 2-year part-time trainee programme is to train primary care doctors to practise Family Medicine at an enhanced level to meet the needs of the child, the adolescent, the adult and the elderly. The course emphasises on basic clinical, diagnostic and management skills essential to general practice. The programme is planned to accommodate the busy doctor’s schedule as almost all courses that require in-person attendance are conducted outside regular office hours. The course consists of the following: l 8 modules of Family Medicine Modular Course (FMMC) with each comprising of 4 workshops, 1 tutorial, online case studies and multiple choice assessments l 3 Practice Management Courses l 3 Compulsory + 1 elective Family Practice Skills Course The examination is conducted by Division of Graduate Medical Studies (DGMS), National University of Singapore (NUS).The standard set is a pass in all three segments: l Written paper I - Applied Knowledge Test: 100 MCQ questions (2 hours) l Written Paper II - Key Features Problems (KFP): 10 questions (1 hour) l Skills Assessment in the Objective Structured Clinical Examination (OSCE) - 10 stations of 11 minutes each, comprising 3 minutes for reading and 8 minutes for assessment (2 hours) The GDFM(NUS) is a title or qualification approved by the Singapore Medical Council for display by Registered Medical Practitioners. The 17th GDFM Examination was held on 21 June 2019 for the written papers and 29 June, 30 June & 6 July 2019 for the OSCE. Family Medicine Residency Programme The Family Medicine Residency Programme aims to equip the resident with educational and clinical experiences in accordance with ACGME-I standards, which focuses on structured training and close supervision. The resident will undergo clinical rotations in the hospitals and polyclinics, call-back sessions and teaching sessions that combine foundation teaching, continuity clinics, skills workshops/ attachments and community medicine. MMed(FM) College Programme The programmeis a structured training programme designed for mature practising family physicians in Singapore. The programme is jointly organised by the College of Family Physicians Singapore and the DGMS, NUS.This 1-year part-time course provides a comprehensive and structured training programme for doctors working in primary care who have completed the 8 modules of the Family Medicine Modular Course (FMMC) to prepare them to sit for the MMed(FM) examinations. The course participants from both the Programmes sit for the same examination – namely, the MMed(FM) examination conducted by the DGMS, NUS. This examination consists of: l Written l 250 multiple-choice questions l Clinical l Logbook-based viva assessment l Slides-based short answer questions l Clinical Skills Assessment Stations The 29th Master of Medicine (Family Medicine) Examination was held on 13 July 2019 (Written), 17 October, 2, 4, 6 and 9 November 2019 (Clinical). FELLOWS OF THE COLLEGE OF FAMILY PHYSICIANS [FCFP(S)] The Fellowship Award by assessment is the highest qualification for family physicians in Singapore. This programme was started in 1998. Potential candidates for this award must successfully complete a demanding two year assessment programme and an exit viva voce examination. Duties and Responsibilities a) To review financial reports from the ED and Secretariat staff. b) To review budgets prepared by Secretariat staff and the various committees. c) To ensure regular and accurate monitoring and accountability for funds, and report to the Council on any financial irregularities and concerns. d) To make financial recommendations to the Council, and to provide feedback on any financial matter specifically designated by the Council. e) To oversee short and long-term investments of the College. f) To advise the ED and other appropriate staff on financial priorities and information systems, depending on committee member expertise. Observations and Decisions by the Committee for the Financial Period under Review The committee advised the secretariat on the fixed deposit placements in terms of the selection of the tenure and the financial institution. Fixed deposits were placed with: l Bank of China l CIMB l HL Bank The committee, upon reviewing the Governance Evaluation Checklist of the Charity Portal, refined the current procedures for handling Conflicts of Interest. The framework for Sponsorship of Overseas Travel was also refined. Both these documents were discussed and approved by the Council.
